Fill in Part 1 and the Screening Questionnaire on the second page. Take the form to your doctor to fill in Part 2 and have the form faxed to our clinic.

Once your referral has been received, our clinic will be in touch to arrange an intake appointment with one of our team members. You may meet with a social worker, an occupational therapist, or a geriatric psychiatrist.
